- Please provide samples of all handouts that will be available at your display.

- Completed form must be returned with $25 per table fee to the Convention Chairman no later than July 15, 2008.

- After review, you will be notified of approval and given instructions for the day of convention.

- Electrical outlets will be provided if possible.  Not all displays will have access to electrical outlets.
